Fill out Parent Financial Statement form (PFS) available online from NAIS using your 2006 tax information. Write all amounts in US dollars. Explain all circled items.
- Complete the Business/Farm statement, if applicable.
- Make two photocopies of the completed PFS form.
- Mail original PFS and check to School and Student Service for Financial Aid.
- Mail copies of your 2006 income tax forms, student tax forms (if applicable), business and farm statement (if applicable), W2s and PFS form to CVA Admissions Office.
- Keep copies of PFS and any other documents for yourself.
- We highly recommend that you request the Family Report from SSS, which gives you the results of the calculation.
